*WWC PAYMENT POLICIES:
- A deposit of $395.00 is required for all new patients at the time of booking your Initial Consult and can be paid via credit card over the phone. It is refundable (minus a $50 non-refundable administrative fee) if you wish to cancel your Initial Consult with 3 business days notice. Sorry, we do not accept cheques by mail for deposits.
- A second payment of $495.00 will be due at your Initial Consult. This is for your Meridian Valley test and interpretation of results
- The remaining balance of $710.00 will be due at your Meridian Valley appointment.
- Payment plans are available (administrative fees apply).

CLINIC SERVICES
ANNUAL MIDLIFE HEALTH PLAN:
The Annual Personal Midlife Health Plan has been developed over 5 years of clinical practice by our physicians with input from our multidisciplinary team of allied health professionals. All our clinicians have special education and experience with women and midlife health and lifestyle transitions. The Plan focuses on quality of life today and prevention of more serious health problems in the future. Our aim is to offer patients truly personalized care and education, guiding women to make informed choices about their health management.
Our niche is "functional" medicine. We look at nutritional, hormonal and emotional/social factors in your life that all add up to factors in midlife health. Our medical model is based on "wellness" and not "illness". We are holistic in the sense that we look at the body as a whole and use a multidisciplinary approach to treatment that includes assessment, testing, understanding, and education We also focus on creating a dialogue between patient and physician.
In order to provide this level of preventative and educational health care for our patients, the clinic offers services on an Annual Plan basis. The Annual Plan is $1600.00* for one year and includes the following:
Initial Consult:
One hour consultation with the physician which includes a review of your personal medical history, a complete physical exam, risk assessment on a wide range of factors such as osteoporosis, breast health, emotional balance and stressors, diet, activity level and more. This is a one-on-one consultation with your physician who will assess your risk factors, unique concerns and needs and explore preventative strategies.
Meridian Valley Hormone Test:
This includes the 45 minute appointment to review your results with the physician. This 24-hour ComprehensivePLUS Profile Urine Analysis from Meridian Valley Laboratory (in Washington state) is considered the gold standard in analysing hormonal imbalances and provides our physicians with leading-edge information regarding your personal hormone balancing requirements. These factors are analyzed and the physician, working together with each patient, offers tools for symptom management, lifestyle modification and wellness. This extensive assessment is recommended for anyone suffering from multiple hormonal symptoms or who is interested in hormone replacement therapies. Regular Check-ups:
Every 3 months, you will have a 30 minute in-office appointment to review your symptoms and progress with your physician. You will also receive prescription refills if needed. Quick 10 minute appointments may also be booked at the discretion of the physician for review of diagnostic test results as ordered by your physician or if you are experiencing symptoms that require medical attention.
Physician Educational Seminars:
These seminars are complimentary in your annual plan and they are an integral part of your care at WWC. Our physicians emphasize the importance of empowering patients to be advocates for their own health through education. These seminars are led by our physicians and they also include seminars with our Allied Health team (herbalist, physiotherapist, relationship counselor).
Educational Patient Binder:
WWC's aim is to take patients beyond healthcare to self-care through awareness and education. As part of the Annual Midlife Health Plan, each patient will receive an extensive take-home binder filled with midlife health information and customized for her particular risk factors and lifestyle. Patients will be given copies of all lab results and diagnostic investigations as well. Our Personal Midlife Health Plan binder includes chapters on topics such as The Menopausal Transition, Sexual Health & Libido, Breast Health, Heart Health, Weight Management & Nutrition, and much more!

ANNUAL HEALTH PLAN RENEWAL
After your first year, you may enroll for subsequent years as needed. You will have a one hour "Reassessment" by your physician, 4 half-hour check up appointments, along with the same services as your first year.

ONE HOUR CONSULTATION ONLY
This option is for patients who may have general questions about their midlife health concerns and bio-identical hormone replacement therapy or to help them decide if the annual health plan is right for them. This option includes a physical exam, review of medical history, 1 Biospace read, management recommendations for follow-up with your family doctor or other practitioner such as a naturopath, an educational binder, and access to one of our 3 hour physician-led Signature Series workshops. Please note that prescriptions cannot be provided under this option as the physician cannot prescribe hormones without the Meridian Valley test.

MALE 1.5 HOUR CONSULTATION + MERIDIAN VALLEY TEST
Due to popular demand, we now offer consultation and testing for male patients interested in addressing midlife health concerns relating to andropause. This option offers an Initial Consult, much like the women's Midlife Health Plan, but focuses on addressing andropause (also known as the "male menopause"), prostate health, cardiovascular health, libido, stress management, diet and lifestyle, and much more. Male patients also receive Male Midlife Health Educational portfolio. Male patients are required to do their Meridian Valley test prior to their Initial Consult. An additional screening blood panel will be billed privately to male patients. Our male patients also have the option of enrolling into an Annual Midlife Health Plan. Options can be discussed at your appointment.

The annual preventative program of risk assessment, integrated medicine and comprehensive hormone testing and education is not covered by MSP

Medically necessary follow-ups and/or diagnostic tests can be billed directly to MSP

Referrals to specialists can be arranged if they are medically necessary and are relevant to your care here at WWC.

This preventative health service may be included in some extended health care packages. Ask your insurance company about "physician-based risk assessments" or "wellness coverage".

We do not provide primary care services - please see your family doctor for your general health care concerns.

We do not provide on-call or emergency services. Please go to your nearest ER deparment or contact your GP.

Our physicians do not provide services for disability or unemployment claims and other similar services requiring the completion of forms. Please see your GP for these services.
